ATTENDING YOUR APPOINTMENT
One carer may accompany patient if assistance is needed
Please bring your Medical card/PPS number and have your phone number to hand when attending.
You must wear short sleeved shirt or blouse under your coat
Some patients will be observed for at least 15 minutes post vaccination
You will receive an appointment for your second vaccine dose on the day of the first
